Whether it’s online or in-person, we’ll need to get some information about you before we can meet. After completing this form you will be directed to our booking site where you can select your appointment time. Tone is a direct care practice so please note that payment for your care is due at the time of booking. Appointments range in cost between $100-$250 depending on your skin & hair concern. Once you are entered into our system, you will receive an email with instructions on how to electronically sign our medical consent forms. We’re here if you have any questions along the way!
